Many patients believe that transferring the embryo during the fresh (retrieval) cycle optimizes their chances of pregnancy. This was true in the past, but with improvements in cryopreservation (vitrification), we find that the success rate following transfer of frozen blastocysts is equal to or exceeds the rate that we have seen with transfer of fresh embryos. We attribute the lower success rate with fresh embryo transfers to the unnatural uterine preparation that occurs with highly elevated levels of gonadotropins and steroids during the cycles for oocyte retrieval.
